Methylated Folate in Pregnancy - Safety, Benefits & Dosage

During pregnancy, the need for folates increases significantly. Methylated folate (5‑MTHF) is an active form of vitamin B9 that does not require conversion in the body and may be better absorbed, especially in women with MTHFR gene variants. This article discusses the safety, mechanism of action, benefits, and dosage recommendations of methylated folate during pregnancy, based on scientific studies and medical guidelines.

1. What is methylated folate?

Methylated folate, also known as 5‑methyltetrahydrofolate (5‑MTHF), is a biologically active form of folate. Unlike synthetic folic acid, 5‑MTHF does not require conversion in the body, which increases its bioavailability and effectiveness, especially in people with limited enzyme activity for folate conversion.

Research shows that 5‑MTHF supplementation raises plasma folate levels faster and more effectively than folic acid, which is particularly important in early pregnancy. (MDPI, 2025)

2. Role of folates during pregnancy

Folates play a key role in fetal development, including:

  • participation in DNA synthesis and cell division,
  • reducing homocysteine levels, which lowers the risk of pregnancy complications,
  • decreasing the risk of neural tube defects such as spina bifida.
“Adequate folate levels before and in the first weeks of pregnancy are crucial for reducing the risk of neural tube defects.” – MDPI, 2025

3. Safety of methylated folate

Clinical data indicate that 5‑MTHF is safe for pregnant women when taken in recommended doses. It does not require liver metabolism, reducing the risk of accumulation of unmetabolized folic acid in the body.

The European Food Safety Authority (EFSA) considers exposure to L‑methylfolate at typical supplement doses safe for pregnant women. (CanABaby, 2023)

Safety recommendations:

  • Avoid taking very high doses of folates from multiple sources without consulting a doctor,
  • Monitor vitamin B12 levels to avoid masking potential deficiencies,
  • In rare cases, mild gastrointestinal discomfort may occur.

4. Benefits of methylated folate

5‑MTHF may provide additional benefits, especially for women with limited ability to metabolize synthetic folic acid:

  • more effective increase in blood folate levels,
  • reduction of homocysteine, potentially lowering pregnancy complication risks,
  • useful for women with MTHFR gene mutations, who have limited folate conversion.

5. Dosage of methylated folate

Dosage recommendations for methylated folate supplementation:

  • 400–800 mcg daily before planned pregnancy and in the first trimester – standard dose for most women,
  • for women with a history of neural tube defects or MTHFR variants, doses may be higher, up to several mg daily, always under medical supervision,
  • supplementation should be supported by a diet rich in natural folate sources: green leafy vegetables, legumes, avocado, nuts.

6. Summary

Methylated folate (5‑MTHF) is a safe and effective form of folate during pregnancy, particularly for women with MTHFR gene variants. Regular supplementation at recommended doses supports proper fetal development and reduces the risk of neural tube defects. Supplementation should be guided by a doctor or nutrition specialist, and the diet should provide natural folate sources.
